5Essential Survey

Benjamin School District 25 Parents,

Your input is an important part of what makes our school district great.

Here is information about our annual 5essentials survey that we ask students, staff and parents to complete.  Please take a few minutes of your time to give us your feedback.

The Illinois 5Essentials Survey is open from February 3rd to March 13, 2026. We are asking parents and guardians to fill out the survey online. This survey is a comprehensive, evidence-based tool designed to drive school improvement. The survey will generate a detailed picture of the inner workings of our school. As a parent, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school. The survey takes about 10 minutes to complete.

The survey gathers data on five indicators that can predict important student outcomes, including improved attendance and more significant test score gains. The five indicators that affect and predict school success are:

  •     Effective Leaders

  •     Collaborative Teachers

  •     Involved Families

  •     Supportive Environments

  •     Ambitious Instruction

Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. Your identity and survey responses will be confidential and never connected to you or your child.

Please let me know if you have any questions.
